F5: Advancing better care for older adults: an IHI global journey

Shifting demographics are accelerating the urgency to design health systems that work for older adults worldwide. This session explores the Age-Friendly Health Systems movement, a growing global effort to deliver care that aligns with an evidence-based framework known as the 4Ms: What Matters, Medication, Mentation, and Mobility. Originating in the United States, the movement has improved care for over 5.4 million people and is now inspiring adaptation in diverse contexts worldwide. Join movement leaders and improvers from Ireland and the US as they share strategies for implementing the 4Ms across settings including hospitals, long term care facilities, outpatient services and more. We will spotlight Mowlam Healthcare’s work in Ireland to embed Age-Friendly care within 20 long-term care facilities. This workshop will offer concrete examples, highlighting shared learning across borders, and provide inspiration for health system leaders seeking to create equitable, person-centred care for older adults in their own regions.
After this session, participants will be able to:
- Understand the critical role the 4Ms framework plays in a variety of settings including LTRC.
- Have an insight into how the 4Ms might be implemented at scale.
- Consider how the participants might transfer the lessons learned to their own health systems.
